Baltimore City Death Certificates 1875 \u2013 1949 will be sent as a JPG and cropped. The request must include name at death, date of death, county of death, and certificate number (deaths in Maryland Counties 1898 \u2013 1968, we do not need the certificate number). For orders of more than 5 Certificates, the price is $2.00 each. Limit 20 death certificates per request.
